Cá thu mua về làm sạch, bỏ ruột và đầu cá, cắt cá làm đôi xếp vào nồi đất. Thêm vào nồi cá gồm: 3 muỗng canh nước mắm, 1.5 muỗng canh đường, 1/2 muỗng cà phê muối, 1 muỗng cà phê hạt nêm, 1 muỗng cà phê tiêu và 1/2 muỗng canh ớt khô xay. Thêm hành tím cắt nhỏ, vài trái ớt khô và cà chua thái nhỏ múi cau.
Cho nồi lên bếp, đậy nắp, đun nhỏ lửa cho đến khi cá sắp chín thì thêm hành lá thái khúc vào, đậy nắp đun vài phút rồi tắt bếp.
